Practice Pro - How to create Custom Record Templates for Client Intake

Custom Record Templates are utilized within the Client Intake module to provide custom fields based on the types of matters the firm handles.

Only users with security access can create or modify these forms.

From the Home screen, click your username in the top right corner.

In the drop down menu, select Firm Settings.

Click Custom Records to filter down to those that will be available within the Client Intake module.

Select the All Filters button.

Use the drop down to change Client Intake from All to Yes

Select Apply Filters

You will only see previously created templates for Client Intake.

These can be edited in the future by clicking on the corresponding pencil icon to create a new one.

Select Add Custom Record.

Give it an easily recognizable name.

Optionally, enter a description of what this template will be used for.

Then start building questions by clicking Add Field.

The text option let's users type in free form responses.

The select list requires them to choose from a predefined set of options.

The Yes slash No field restricts the response to either Yes or no.

The Date field ensures the entry is in date format only, and then there are additional options within the drop down including requiring the response to be an e-mail address, phone number, currency number, link, etcetera.

Any changes will be saved automatically and you can always click the preview tab to see how it will look to users.

This custom record template will now be available within the Client Intake module when filling out prospective client information.
